Another information packed day at the conference.  Brian Behlendorf of NHIN Connect, Fishetti & Tastrom on VA's Innovations program, Dr. Howard Hays on IHS' Meaningful Use efforts, Jim Hebsleb of CMU with an update on the study of the open source VistA community, Rob Kolodner of OHT and Collaborative Transformations spoke of VistA (and HIT) in a changing world.

 

The 'keynote' for today was by Roger Baker, CIO of the VA (Scott Cragg also joined him).  He has been in the position for just over a year and provided the group with an update on his initiatives, philosophy and the status on a number of fronts.  I jotted down a few notes as well:

 

  • VA IT Budget is $4B
  • He has spent time looking at the development process
    • One myth he found was that 'centralization' was what killed VistA innovation -- he felt stagnation had occurred long before
    • In order to improve things, he's started a number of initiatives
      • Customer Forward Development = moving developers back into the clinical facilities
      • Sandbox environments
      • Innovation Initiatives (see Fishetti and Tastrom's discussion)
  • Private Sector is catching-up
    • VA will consider COTS where it makes sense
    • COTS will surpass VistA in 5-15 years
  • VA has other things to offer -- its research in health care improvement adds huge value
  • ... on the IAC Report:
    • There was good and bad
    • He appreciated the input
    • ... and welcomes reasoned, thought out opinions
    • Sept 1 is the deadline for input, he is targeting Dec 1 as the deadline for a plan to reach a decision.  He suggested that is probably aggressive and may slip
  • He acknowledged the problem w/ the FOIA process and FTP site.  Scott Cragg has the authority to get it fixed.

A number of great talks during day two of the meeting.  The highlights were to hear Dr. Douglas Rosendale talk about interoperability between DoD and VA and Conor Dowling talk about Semantic Web EHRs.

 

Andy Pardue (remotely) and I talked about Medsphere's platform technologies, OVID and FM Projection, in our "Extending the Platform" presentation.

 

We took the opportunity to take an in depth look OVID's capabilities, providing some examples of use, some recent projects the technology was used in, and a live demonstration.  In particular OVID's use as a means to build upon OpenVista (and VA VistA), providing a layer on which to abstract the existing platform. This new layer opens the doors to a larger pool of contributors and possible innovations.  To learn more, take a look at a recent whitepaper on OVID.

 

FM Projection is a tool that "projects" the OpenVista (or VA VistA) data structures through a MySQL instance, allowing SQL style queries and broader reporting tools.

Today was the first day of the VistA Community Meeting, hosted at George Mason University (go Patriots!) in Fairfax, Virginia.  Participants were treated to presentations on several ongoing implementations (India and Puerto Rico) as well as provocative talks by Phillip Longman (Best Case Anywhere) and Ed Meagher (IAC chair of the VistA Modernization Committee).

 

I presented an update on the various projects and releases currently available here on Medsphere.org:

Extending the Platform

Andy Pardue, Jeff Apple and I had an opportunity to present some of the platform technologies that are a part of the OpenVista stack at the VCM today.

 

  • Andy walked through the capabilities of the OVID technology, including capabilities, contributions and important tutorials
  • Jeff talked through the newly released FM Projection technology from an architecture and status perspective
  • I took the final portion of the presentation and demonstrated FM Projection in use (including FM Visualizer, SQL queries in DBVisualizer and finally some reporting in Jasper's iReport tool)

Today was the fist day of the 20th VistA Community Meeting (VCM) in Tempe, Az.

 

Edmund Billings was given the opportunity to speak on the topic of VistA strategy, partnering with the client and Meaningful Use.  I think it is fair to say his presentation was well received and generated a good discussion within the audience.

 

Jon Tai and I presented an update on Medsphere.org and the various open source projects and releases.  The big announcement of the day was the release of FM Projection -- a technology that allows us to project FileMan structures into SQL structures (not replication or conversion) for querying via standard tools.  A recording the presentation was captured and is available:

As OpenVista proliferates in academic environments through Medsphere's Academic Incubator program, Medsphere wants to facilitate a dialogue among all participating schools that enables the sharing of information. As an open source project, Medsphere knows the absolute necessity of collaboration in making OpenVista the solution that powers clinical transformation and healthcare facilities nationwide, and that prepares students for the future as the face of healthcare dramatically changes. This initial call will introduce participants to one another, and bring the group up to speed on what the early adopters have done and where we can take this project moving forward. A core benefit of electronic health records (EHRs) is the ability to capture data related to patient care and then sift through that data for patterns and trends. Searchable data is a necessity for generating best practices and improving existing workflows. Of course, the benefits to a single hospital are greatly expanded when data is collected among several facilities, including clinics. Regional health information exchanges (HIEs) enable administrators, care providers, and other personnel to make decisions with a knowledge of what works, thereby improving patient care.

FOSSHealth 2009

Andy Pardue and I just finished presenting at FOSSHealth 2009.

 

This is the first annual "FOSSHealth unconference" organized by Fred Trotter and Liberty Medical Software Foundation.  A big thanks to Fred for allowing us to present and to put up with the hassle of a remote presenter (and all the technology challenges associated with that -- more on that in a moment).

 

At 2pm Central, I presented the Medsphere Overview presentation, which focuses on the company's foundings, team, goals of Clinical Transformation, Ecosystem strategy and tangible results from both a clinical and financial perspective.  It wouldn't be a presentation without a mention of "Stimulus 1", Medsphere's bus (in fact, I don't know if I would have a job come Monday if I didn't mention it).  The presentation is attached.

 

At 3pm Central, Andy and I presented the Medsphere.org: Released slide deck.  This presentation provides a brief history of the website and future direction. We then talk through each major software project hosted on .org, providing vital statistics.  When we reached the section on OVID, Andy took over and drilled into the details of OVID -- what it is and can do, how it works, why it's important (at least to us), as well as two code tutorials (both of which are available online here and here.)

 

Unfortunately right at the end, we ran into our first and only network problem (namely my connection), which means participants couldn't see the final few slides, nor could they ask any questions.  If you attended and want to "see" what I was showing, the presentation is attached.  Likewise, if you had questions -- you can drop them here or in the specific project space, if applicable.

Community Call, July 2009

Information technology is clearly necessary but not sufficient to attain Meaningful Use.  Success also requires leadership at all levels, knowledge of best practices, implementation of content that supports best practices, and direct incorporation of practices into clinical workflow. A genuine understanding of improved outcomes demands re-evaluation and measurement over time.

 

While some healthcare organizations have achieved these improved outcomes, the level of effort and cost required varies far and wide.  In the spirit of open source collaboration, Medsphere believes the community should work together to find the most efficient path to meaningful use and better medicine.

 

Medsphere has focused on these and similar topics for some time and feels that our research and findings can inform community discussion and provide a starting point from which to achieve crucial Meaningful Use goals.

This month's community call will focus on the topic of the ARRA/HITECH stimulus bill from the perspective of improving clinical outcomes.  Of course, to properly address this topic we must also review the economic and fiscal aspects of the legislation.

 

This month's topic would be interesting for healthcare facility's financial roles such as CFO, Director of Finance, and Controller.  Additionally, executive staff involved in the business aspects of running a hospital, clinic, FQHC, CHC, or private office.

 

Further, we will look at the "meaningful use" provisions of the bill, which is a topic all providers should be aware of as they review, evaluate and select systems over the next few years.

When integrated medication processes and automated cabinets are in place, pharmacists are enabled to practice more “clinical pharmacy”.  Often this means getting pharmacists out of the basement and onto the floors where they are available to patients and their caregivers.  One of pharmacy’s services is to promote a safe, effective, and economical  list of preferred drugs. With well-designed EHR technology as a tool, pharmacists can proactively influence physicians to choose the “right” drug. They can also measure and report on compliance to formulary preferences. The goal of this session is to explore the options available and present experience with a program currently in place at one of our ecosystem sites.

 

This topic is both clinical and administrative in nature and will likely be useful to all pharmacy staff, clinical specialists involved in building/maintaining CPOE systems, physicians, nurses and others interested in pharmacy management, both from a clinical and fiscal perspective.
